Introduction

“Human error” is ascribed as the cause of incidents, whether its injuries, unit trips, equipment damage, environmental releases, etc. How can we minimise events caused by human failures? For long, there has not been a good answer to that question. Understanding the complex nature of human error educates us how it can be managed in a better way.
